Wholesale Account Registration

If you are a health practitioner and have not established your wholesale account, please follow the these 3 easy steps to establish a Wholesale Account:

  1. Please download the Practitioner Registration Form (PDF), print, complete the requested information (black ink), sign and FAX to 1-866-819-4038.
  2. Register for a wholesale account by clicking Register Now *Remember to select Wholesale in the Signup for Membership field.
  3. Wait for an email indicating that your application for wholesale membership has been reviewed (typically one business day).
